Dental insurance is designed to help reduce your out-of-pocket dental expenses, and you will usually be expected to pay some out-of-pocket expenses at the time of service. Your insurance provider determines reimbursement levels and generally does not consider up-to-date regional data when setting them. Additionally, there is no regulation governing how insurance companies determine reimbursement, and they are not required to disclose how they set these levels. Insurance companies do not guarantee payment on claims and reserve the right to make payments based on their estimation of "Usual and Customary Rates."

A written diagnostic report should be provided if your insurance company has any questions about the services provided.
